2 grandchildren: Adalynn Marie Perkins and Isaac McCoy Perkins, both of Greensburg
2 brothers: David Moore of Corbin, Roger Moore of Michigan
4 sisters: Kim Sturgeon of Cave City, Lisa Elmore of Hodgenville, June Clem of Harlan, and Susan Maggard of Harlan

Funeral Service: 1:00 PM, Tuesday at Cowherd & Parrott Funeral Home
Burial: Macedonia Cemetery
Visitation: 2:00PM until 8:00PM Monday and after 7:00AM Tuesday until time of the service
