Hazel Marie Rakes, 26, of Campbellsville, KY, the daughter of Robert Lee Rakes and the late Jennifer Gail (Tungate) Rakes, was born November 9, 1998 in Campbellsville and departed this life on Sunday, October 5, 2025 in Greensburg at the Jane Todd Crawford Hospital as the result of a motorcycle accident. 

She enjoyed hunting and fishing with her dad and spending time with her children.

3 children: Jessica Hope Smothers, Oliver Ellis Hall, and Anderson Blake Hall

Her grandparents: Linda Lou Rakes and Judy Tungate

Funeral Service: Thursday, October 9, 2025 at 1:00 P.M. in the chapel of L.R. Petty Funeral Home in Campbellsville with Elder Jimmy Sharp officiating

Interment: Jesus Name Church Cemetery

Visitation: After 11:00 A.M. Thursday until the time for the service
