Please bring your kindergarten child for a quick kindergarten readiness screening and bring the following documents for enrollment:

• Birth Certificate (must be 5 by August 1^st to enroll for kindergarten)
• Social Security Card
• Immunization Record
• Physical Exam
• Dental Exam
• Eye Exam &
• Custody Documents (if applicable)
